Treatment for an Infected or Damaged Tooth

Root canal treatment addresses inflammation or infection in the soft tissue inside a tooth, known as the pulp. This tissue contains nerves and blood vessels.

Symptoms can include toothache, sensitivity or swelling, although some affected teeth have few symptoms. A dental examination and appropriate imaging help identify the cause.

Treatment aims to remove damaged or infected pulp, clean and seal the root canals, and retain a tooth that can be restored. It cannot save every tooth.

The pulp may be affected by:

  • Deep decay.
  • A crack or chip extending into the tooth.
  • Previous extensive dental work.
  • An injury to the tooth.

After the canals are treated, the tooth needs a suitable filling or crown to restore and protect it. Your dentist will explain the number of visits, alternatives and the prospects for your tooth.

Extraction may be an appropriate alternative when a tooth cannot be predictably restored, or if you choose not to proceed with root canal treatment. Before deciding, discuss:

  • Whether the gap could affect neighbouring teeth or your bite.
  • How losing the tooth may affect chewing.
  • Whether replacement and ongoing maintenance will be needed.

An implant, bridge or denture may be considered for the space, depending on your oral health and preferences. Compare the likely outcomes, stages and total costs of retaining or replacing the tooth.

Treatment may require more than one appointment. Your dentist will explain the planned sequence, which generally involves:

  1. 1. Numbing the tooth and isolating it with a dental dam to help keep the treatment area dry.

  2. 2. Removing the affected pulp and cleaning and shaping the root canals, with imaging as needed.

  3. 3. Filling and sealing the canals when ready; a temporary filling may be used between appointments.

  4. 4. Restoring the tooth with an appropriate filling or crown after the root canal work.

Some discomfort can occur afterwards. Persistent infection, damage to the root or an instrument complication can mean further treatment is needed. Contact the practice if symptoms worsen.

The final restoration is an important part of care. A crown may be recommended, particularly for a back tooth with substantial structural loss; your dentist will explain why it is appropriate for your tooth.

Continue brushing twice daily with fluoride toothpaste and cleaning between the teeth. Follow any specific instructions while a temporary filling is in place and arrange the recommended final restoration.

A root-treated tooth can still develop decay, gum disease or recurrent infection. The tooth or restoration may also fracture. Contact your dentist if you notice pain, swelling, a loose restoration or a change in your bite.

Regular reviews help assess the tooth and surrounding tissues. Your dentist will recommend a follow-up schedule and any imaging needed to monitor healing.

The cost depends on the tooth, the number and complexity of its canals, previous treatment and the appointments needed. Ask whether the final filling or crown is included in the estimate.

Complex cases may be referred to an endodontist, a dentist with specialist registration in endodontics. Any referral and separate fees will be discussed with you.
